PeopleSoft GL User ID Form.doc will open automatically in Microsoft Word. The applicant should complete the form in Word, print it, sign the form, obtain the supervisor's signature, and send the original copy to Carolyn Livingston, Director of Financial Information Systems.
For audit purposes, FIS needs the original copy.
